Employee failed to wash hands before putting on gloves to initiate a task working with food. Observed employee wiping the counter-top in front of the cookline without glove, employee grabbed clean gloves and put them on his hands without washing hands between tasks. Inspector coached the employee on when to wash hands between task. Employee removed/discarded the gloves, washed hands properly, and put on new clean gloves.
No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to food borne illnesses. Discussed with operator and provided form.
Equipment in poor repair. Reach in cooler with the pull-out drawers in front of the cook line: gaskets on both drawers torn.
