Employee wearing jewelry other than a plain ring on their hands/arms while preparing food. Observed employee with bracelet on while engaging in food preparation, employee took bracelet off.

Water with a temperature of least 85 degrees Fahrenheit not provided/shut off at employee handwash sink. Hand washing sink on the right side of the front counter area with a hot water temperature of 74F. Manager adjusted the hot water new reading 91F.

Employee failed to wash hands before putting on gloves to initiate a task working with food. Observed an employee putting on gloves after touching door and not washing hands, operator corrected asked him to stop and wash hands.

Non-food grade paper/paper towel used as liner for food container. Observed paper towels in a spinach container lining in between the spinach to dry it, operator removed the spinach.
Water with a temperature of least 85 degrees Fahrenheit not provided/shut off at employee handwash sink. Observed three handwashing sinks with no hot water, operator stated they just put in three new hot water heaters, the two and three compartment sinks have hot water and the dish machine has hot water. Operator corrected by turning on hot water.
Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed four vents above prep area in the kitchen near the three compartment sink soiled.
No conspicuously located ambient air temperature thermometer in holding unit. Reach in cooler on the front line does not have thermometer, operator placed one in there.

Single-use gloves not changed as needed after changing tasks or when damaged or soiled. Observed cook at makeline leave line and open cooler at front counter where cashier opens cooler with no gloves after touching money. Cook opened cooler, unsanitized surface, and returned to makeline where he handled ready to eat foods touching and wrapping sandwich without first changing gloves and washing hands. Manager reviewed handwashing and glove use with employee.
No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. At inspection one employee hired 6 days ago had not signed the health reporting agreement. Manager had employee sign during inspection.
Unwashed fruits/vegetables stored with ready-to-eat food. Observed unwashed melon over cut ready to eat melon in walk-in cooler. Manager rearranged shelves
Vending machine exterior soiled. Observed grey growth behind nozzles of drink dispenser. Manager cleaned with sanitizer.
Working containers of food removed from original container not identified by common name. Observed spray bottle of water with no label. Manager labeled.

Employee with no beard guard/restraint while engaging in food preparation. Observed line cook with no beard guard. Manager had employee apply beard guard.
In-use ice scoop stored on soiled surface between uses. Observed ice scoops stored in container with buildup of bust and debris. Manager removed all to dishwash area.
