Incorrect/ineffective/inaccurate chemical test kit provided for measuring the concentration of the sanitizer solution used in the three-compartment sink and/or dishmachine. Manager provided chlorine test strips. Strips provided will measure chlorine in the dishmachine to only 25ppm chlorine according to color concentration chart provided. Provided test strips were not past expiration date. Inspector's chlorine strips were used to verify proper concentration and tested twice at 75ppm chlorine.

Floors under drive thru soda machine, drive thru register counter, under and around bag in box soda rack, in walk in freezer and under all flip top reach in coolers in kitchen have debris and/or soiled. Multiple vents and surrounding ceiling tiles on make line and vents above reach in cooler across from back prep area are dusty. Fan guards in small walk in cooler have debris.

Dish machine tested at 0ppm chlorine. Operator made adjustments to machine and retested at 50ppm chlorine.
Employee failed to wash hands before putting on gloves to initiate a task working with food. Observed employee cleaning counter at drive thru area, then put on gloves without washing hands to prepare a food order for a customer. Inspector spoke to employee. Employee removed gloves, washed hands and placed on new gloves to continue food prep.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Cooked chicken in drive thru reach in cooler at 45F. Per employee, chicken placed in cooler approximately two hours prior. Operator moved chicken to walk in freezer. Temperature retaken at 38F.
Spray bottle containing toxic substance not labeled. Unlabeled spray bottle with clear liquid on wall rack near mop sink not labeled. Operator removed bottle from supply.

Observed four employees with inaccurate issue and/or expiration dates. Three have expiration dates of four years from issue date and one has expiration date of six years from issue date. Operator states either system issues or manual entries may have caused the errors.
