Paper towel dispenser at handwash sink not working/unable to dispense paper towels. Observed at the hand washing sink at the end of the front counter a paper towel dispenser that is unable to dispense paper towels. Operator changed batteries.

Time/temperature control for safety food removed from cold holding for necessary preparation rose above 41 degrees Fahrenheit during the preparation process and was not cooled back down to 41 degrees Fahrenheit within 4 hours. Grilled onions 51 F at 9:23 AM, 50F at 10:15AM, in flip top cooler. Manager placed in walk in cooler to rapidly cool.
