Observed dishwasher handle dirty pans, then begin to put away clean dishware without hand wash. Spoke to employee who washed hands.

Water with a temperature of least 85 degrees Fahrenheit not provided/shut off at employee handwash sink. Hot water at front hand wash sink by counter not functioning. Cold water only at 72F. Person in charge has work order in for sink.

Employee came in from dining area, drank beverage and placed in holder and then placed gloves on without washing hands, then wrapped pan of food. Discussed with operator.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. cut lettuce (47F - Cold Holding); in top drawer of flip top reach-in cooler across from hand wash sink on cook line. Observed drawer not fully closed because. Of gasket hanging off at bottom. Operator removed cut lettuce and placed in walk-in-cooler to chill. Rechecked temp at end of inspection and was at 42-43F
Food-contact surface soiled with food debris, mold-like substance or slime. Portable brown cutting board cover and multiple cutting boards in prep area soiled/ stained.
Cold holding equipment not maintained in good repair. Do not store time/temperature control for safety food in this unit until the unit is repaired. Reach-in cooler on left of kitchen entry next to dairy cooler unable to maintain temperature of 41F and below. Ambient temperature 47-48F. Observed the top drawer of cooler not fully closing because of gasket hanging off. Operator placed gasket properly back on so drawer able to fully close.
