Employee began working with food, handling clean equipment or utensils, or touching unwrapped single-service items without first washing hands. Male employee came from outside and started working with the coffee machine, no handwash occurred, Manager instructed to stop and wash hands.
Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. At the mop sink, splitter added, no backflow preventer installed at the non chemical side of the splitter.
Single-service articles not stored inverted or protected from contamination. Cups at the front counter area not inverted, manager inverted.
Wet wiping cloth not stored in sanitizing solution between uses. One white wet cloth on cutting board at the front line, manager moved back to sanitizer bucket.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Items in reach-in cooler across from coffee station: tuna salad (46F - Cold Holding); turkey (45F - Cold Holding); beef (46F - Cold Holding); feta (45F - Cold Holding). Employee stated items placed in cooler two hours prior to temperature being taken. Manager moved items to walk-in cooler to reduce temperature to 41F.
Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Hose side of splitter in mop sink missing vacuum breaker. Maintenance man added vacuum breaker to splitter.
Food-contact surface soiled with food debris, mold-like substance or slime. Cutting boards stored above the dish machine stained. Water nozzle for tea machine next to three compartment sink.
In-use ice scoop stored on soiled surface between uses. Ice scoop stored on top of ice machine. Manager moved ice scoop to dish area.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Rolling racks used for baking.
Single-service articles improperly stored. To go cups stored on floor in kitchen. Manager moved to go items to shelf.
