Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed pre-made uncooked sausage and cheese pizza (61F - Cold Holding); uncooked chicken and cheese pizza (61F - Cold Holding); uncooked cheese pizza (62F - Cold Holding)on rack by pizza oven. Per manager, pizzas were prepared 5 minutes prior. Discussed with manager time as a public health control and provided required written procedure paperwork and time as a public health control informational sheet to manager.
No proof of required state approved employee training provided for employee hired more than 60 days ago. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. Two employees(John B and Ryan S)over 60 days of employment with no proof of food handler training and one employee, Penny H with 33% completion of food handler training.

Toxic substance/chemical improperly stored. On bottom shelf of prep table across from walk-in cooler, spray bottle of equipment cleaner (orange pro) stored on shelf next to single use bags and wiping clothes. Employee removed spray bottle from shelf.
Ceiling/ceiling tile shows damage or is in disrepair. Ceiling tile in water heater room showing signs of water damage.
Cove molding at floor/wall juncture broken/missing. Cove molding tiles missing to left of triple sink.
Dead roaches on premises. Observed 1 dead roach under delivery driver table. Manager discarded of dead roach in trash can.
Floor, walls and/or ceiling soiled/has accumulation of debris. Wall behind storage self with 2 liters of soda and triple sink soiled. Floor under dough station has food debris.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Gaskets soiled on reach in cooler by pizza oven.
Open dumpster lid. On shared dumpster out back of establishment, lid open on both dumpster. Manager closed lids.
Unclean building components, attachments or fixtures. In walk-in cooler, fan covers have build up of dust.
