Floor soiled/has accumulation of debris. Build up of food debris under racks in back storage area. Build up of debris under tables in corner of office area.
No handwashing sign provided at a hand sink used by food employees. At hand wash station by pizza make line.

Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Ceiling vents over cook line have build up of dust.
Equipment and utensils not properly air-dried - wet nesting. On the storage shelf over the three compartment sink, clean plastic containers stacked while wet.
In-use utensil not stored with handle above the top of time/temperature control for safety food and rim of the container. Scoop handle in shredded cheese not stored above the shredded cheese.
Interior of refrigerator or freezer in disrepair/has exposed insulation. Flip top reach in cooler doors in poor condition, separating when door is opened. Cooler door closest to the hand wash sink has exposed insulation when opened.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Cook line reach in cooler door closest to prep table has build up of flour in the door gasket. Pizza tray stacked mounted to wall by hand wash sink has build up of food debris. Walk in cooler door gasket has build up of mold like substance.
