Employee handled pepperochini and pizza boxes with bare hands. Employee unable to produce alternative operating procedure. Employee discarded pepperochini and started using tongs.
No chemical test kit provided when using sanitizer at three-compartment sink/warewashing machine or wiping cloths. Quaternary test strips on hand are unusable due to mold like substance and apparent water damage.
