Employee touching prepping and plating prepared sandwiches with their bare hands - food was not being heated as a sole ingredient to 145 degrees F or immediately added to other ingredients to be cooked/heated to the minimum required temperature to allow bare hand contact. Establishment has no approved Alternative Operating Procedure.
Observed Non-food grade paper/paper towel used as liner for container of various prepared sandwiches.
Observed the following food temperatures greater than 41°F ham and cheese sandwich (61F - Cold Holding); turkey sandwiches (59F - Cold Holding) All foods were located on front counter and per manager had only been out for about 30 mins.
Observed plastic container and knife stored in Handwash sink behind front counter. Advised all sinks must be accessible for employee use at all times.

Advised manager that all food employees shall clean their hands and exposed portions of their arms immediately before engaging in food preparation including working with exposed food, clean equipment and utensils, and unwrapped single-service and single-use articles.
Observed raw shell eggs stored over/not properly separated from cut lettuce in walk in cooler.
Handwash sink in kitchen/warewash area not accessible for employee use at all times due to cleaning chemicals being stored inside. Manager removed.

Observed Employee washed hands in 3 compartment sink. Advised manager that Food employees shall only clean their hands in an approved handwashing sink.
Observed Water with a temperature of least 85 degrees Fahrenheit not provided and shut off at employee handwash sink behind front counter. Advised manager and he turned on water.
