Observed employee handle broom and dust bin then handled clean equipment from popcorn machine; no hand wash. Advised employee to wash hands and sanitize equipment.

Dead roaches on premises. 3 dead roaches observed beside stovetop in kitchen. 1 dead roach observed on shovel with ice retrieved from ice machine. As per operator, ice was to be used for employees. Employees cleaned and sanitized ice machine and area beside stove.
Food stored in a location that is exposed to splash/dust. Observed under prep table on a shelf, several packets of condiments heavily soiled with dust. Operator discarded.

Wiping cloth quaternary ammonium compound sanitizing solution not at proper minimum strength. Sanitizer Bucket (Quaternary 50ppm). Operator corrected to Sanitizer Bucket (Quaternary 200ppm).
