Standing water or very slow draining water in handwash sink, three-compartment sink, or mop sink. Observed about 6 inches of standing water in Mop sink. Drain does not appear to be draining. Employees stated drain was functioning at shift start. Water is contained in mop sink basin.

Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Ice machine bin at soda fountain has debris buildup under lip of sliding stainless steel lid. Ice machine in kitchen has black substance on the inside left and right sides of bin.
Carbon dioxide/helium tanks not adequately secured. Carbon dioxide canisters in dry storage closet not chained/secured. Manager chained all canisters to wall during inspection.
Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Employee phone on prep area over make line. Manager properly stored phone during inspection.
Food stored on floor. Cooking oil stored on floor in dry storage closet. Manager put cooking oil on stand elevating it more than 6 inches off floor during inspection.
A return visit. The problems were not fixed yet and the kitchen was given more time.

Ceiling tile missing: observed one spot in a back hallway that was missing its ceiling tile.

Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance: observed 27 ceiling tiles throughout the establishment that has water stains on them, eight with a mold like substance on them. Additionally, there were four pieces of a wood ceiling in the south dining area that have water stains on them.
