Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Black substance observed on ice shield in left side ice machine at back of kitchen.

Cooked/heated time/temperature control for safety food not cooled from 135 degrees Fahrenheit to 70 degrees Fahrenheit within 2 hours. Cook to cool- refried beans 113f, ground beef 108f, chicken 112f all 3 hours. Operator instructed to reheating and start cook to cooling process over.
Raw animal foods not properly separated from each other in holding unit based upon minimum required cooking temperature. Upon inspection observed raw eggs stored above raw shrimp in reach in cooler next to cook line. Operator corrected storage during inspection.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Reach in cooler under cook line- pork 44f. Doors to cooler opened often. Operator placed ice to bring temperature down.
Nonfood-contact surface soiled with grease, food debris, dirt, or dust. Upon inspection observed fan screen cover with debris present in walk in cooler.
Old labels stuck to food containers after cleaning. In hall way clean storage rack.
