Cutting board has cut marks and is no longer cleanable. Slicer board is grooved and has black on it.

Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. At outside mop sink, no vacuum breaker after splitter. Hose to chemical dispenser disconnected. Operator reconnected to chemical dispenser.
Handwash sink not accessible for employee use at all times. On cook line, hand sink blocked by speed rack with dough, operator moved from in front of sink.
Equipment in poor repair. In prep area, walk in cooler gasket is torn and in poor repair.
Garbage can located outside has no lid or lid open/broken. Garbage lid broken. Also, side door to dumpster was open during inspection.
In-use utensil not stored on a clean portion of food preparation or cooking equipment. At pizza station, in use pizza paddles stored on top of pizza oven. Top of oven is soiled with grease.

Utensils in poor condition. At pizza station, pizza paddles used for pizza prep/baking are cracked and chipped on edges.
