Employee touched soiled apron/clothes and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. 1. Employee at cookline drank from his drink cup with gloved hands, wiped the gloves on his apron, then continued to handle utensils at the cookline. 2. Employee at cookline scratched her face with bare hands, then placed both hands on hips, then proceeded to handle utensils at the cookline. Manager had both employees wash hands before continuing to work at cookline.
Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Backflow prevention device missing at green hose connection after splitter added to mop sink. Manager removed the splitter during this inspection.
Old labels stuck to food containers after cleaning. Remnants of old labels on sides of several clear plastic pans on clean dish rack.
Open dumpster lid and/or missing drain plug. 1. Drain plug missing from cardboard dumpster. 2. Lids of both dumpsters left open. Manager closed both lids during this inspection.

Manager advised that lasagna had been taken out of walk-in-cooler and stocked on cooking 30minutes earlier. Manager placed lasagna in walk-in-cooler.
