Wiping cloth quaternary ammonium compound sanitizing solution not at proper minimum strength. Observed sanitizer bucket with quatanery ammonium solution testing at 0ppm. Person in charge voluntarily discarded contents and set up new bucket with quatanery ammonium testing at 200ppm.

Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Observed missing vacuum breaker on splitter at mop sink at back door. Person in charge added a vacuum breaker to splitter.
Spray bottle containing toxic substance not labeled. Observed spray bottle with yellow toxic liquid person in charge indicated is sanitizer. Person in charge labeled on site.
Accumulation of lime scale on the inside of the dishmachine. Observed interior of dish machine soiled with mold like substance and with lime build up.
Dead roaches on premises. Observed two dead roaches in storage closet next to restrooms. Person in charge cleaned and discarded.
Drain cover(s) missing. Observed missing drain cover for drain under three compartment sink.
In-use utensil in non-time/temperature control for safety food not stored with handle above top of food within a closed container. Observed scoop in cinnamon sugar container on cook line with handle in contact with product. Person in charge removed the scoop and placed in designated storage area.
Reach-in cooler interior/shelves have accumulation of soil residues. Observed soiled shelves in reach in cooler with dough on cook line by soda cooler.

Hood filters over fryers have grease build up. Multiple green racks/shelves, including on prep tables, walk-in cooler and proofer cabinet, have grease build up/soiled.
