Toxic substance/chemical improperly stored. On top shelf of metal rack next to mop sink, spray bottle of disinfectant and unopened bottle of bathroom cleaner stored next to single service gloves for food. Operator moved chemicals to different shelf with other chemicals.
Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Vacuum breaker missing at the end of the splitter for mop sink faucet where black hose is attached.
Current Hotel and Restaurant license not displayed. Operator unable to provide copy of current license.
Equipment and utensils not properly air-dried - wet nesting. Multiple plastic pans on top shelf of metal rack next to 3 compartment sink and metal pans on shelf under computers stacked before proper air drying.

Interior of oven/microwave has accumulation of black substance/grease/food debris. Interior of pizza oven has black buildup up hanging down from top.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Gaskets soiled with food debris on reach in freezer by fryers, pizza warm holding unit and reach in cooler.
