In-use utensil not stored with handle above the top of time/temperature control for safety food and rim of the container. Scoop handle touching mozzarella cheese in prep area. Operator removed scoop from mozzarella cheese.
Wiping cloth quaternary ammonium compound sanitizing solution stored in prep area not at proper minimum strength. Reading 0ppm. Operator replaced sanitizing solution and re checked 200ppm.
