Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed dust build up on the ceiling vents and tiles in the kitchen prep and cooking area.
Floor tiles missing and/or in disrepair. Observed broken and missing tiles in the prep area and by the oven.
