Employee handled a garbage can then clean equipment without washing their hands. Employee removed the equipment to be sanitized and washed their hands.

Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Observed vacuum breaker is incorrectly installed on the hose Bibb before the splitter and not after the splitter.
Handwash sink missing in warewashing or food preparation area. Observed waitstation for drinks at dining area with ice bin for dipping ice and no handsink available in front area. Manager stated remodel is underway and new plan should be submitted to DBPR soon.

Employee with long fingernails working with exposed food, single-service items or clean equipment and utensils. Observed employee with long fingernails dipping ice from ice machine.
Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed ceiling rails and vent covers on ceiling had been cleaned until paint was removed in some areas. Some mildew like stains still persisting. Operator states contracted repairs are on order to replace ceiling rails and vent covers also looking to add dehumidifier.
Employee wearing jewelry other than a plain ring on their hands/arms while preparing food. Employee with multiple filigree rings dipping ice and working with open foods. Manager had employee remove rings and wash hands before continuing.
Equipment and utensils not properly air-dried - wet nesting. Observed cups and pans stored stacked wet at dish dry rack. Operator I stacked dishes to promoate air drying circulation.

Wall soiled with accumulated grease, food debris, and/or dust. Observed ladder to roof beside dishwash area with grayish green growth on ladder and at wall. Operator called in work order to have ladder area deep cleaned.Observed black growth in drain and under ice machine. Operator states that another ice machine is being brought in and deep cleaning will occurs under the machine at that installation.

Manager states it is in the training but not on first day of hire. Inspector suggested that Pizza Hut contact the state to see if the training provided will fit the requirements needed. Inspector provided handout via email to implement.
Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Manager showed that machine was off in preparation for cleaning and replacing drop guard tonight.
Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed black growth and dust on ceiling air conditioning vents in kitchen at warewash area, also some ceiling tile rails discolored or soiled. Some areas over cookline the vents have been scoured clean with paint scoured off to metal in some areas and stains still persisting within some areas of the paint that remains. Manager resanitized areas at warewash. Manager states that the railings and tiles and vents are under bid to be replaced.
Equipment and utensils not properly air-dried - wet nesting. Observed pans stacked inside each other still wet. Manager rearranged pans to promoate air drying.
