Observed cook exit bathroom and enter kitchen and begin handling single service pizza box no hand wash. Discussed proper handwashing procedures with employee and person in charge.
Wiping cloth sanitizer solution exceeds the maximum concentration allowed. Observed sanitizer bucket at 500 ppm. Employee diluted solution with water final reading 400ppm.
Equipment and utensils not properly air-dried - wet nesting. Observed plastic containers wet nesting on shelf above three compartment sink.
