Vacuum breaker missing/incorrectly installed at mop sink faucet or on fitting/splitter added to mop sink faucet. On mop sink with attached splitter, vacuum breaker is at the top of the splitter. Vacuum breaker absent on both sides of splitter.
Food-contact surface soiled with food debris, mold-like substance or slime. Interior of two right nozzles on soda machine have debris/slime-like substance.
