Employee touching ready-to-eat food with their bare hands - food was not being heated as a sole ingredient to 145 degrees F or immediately added to other ingredients to be cooked/heated to the minimum required temperature to allow bare hand contact. Establishment has no approved Alternative Operating Procedure. Observed employee peeling hard boiled eggs with bare hands. Hard boiled eggs are for service with no additional heating. Employee placed on gloves.
In-use utensil not stored with handle above the top of time/temperature control for safety food and rim of the container. Handles on utensils for raw salmon and shrimp stored with handle touching products. Operator moved both utensils to proper storage.

Operator states tapioca prepared at 12:00. Inspector took temperature at approximately 3:15. Operator opted to hold tapioca pearls on time controls. Inspector provided paperwork and assisted in completion during inspection. Employee marked appropriate time on tapioca pearls.

Also, multiple containers of raw fish on serving line have a paper towel in the bottom of the containers.

In walk in freezer, opened raw chicken stored on top of case of raw tuna. In reach in freezer at triple sink, opened raw beef stored above raw fish and rice cakes. Operator began to rearrange some of the products to proper storage.

Employee with gloved hand placed raw salmon in the bowl then placed various customer choice of vegetables in the bowl with the same gloved hand, no heat applied to meal. Inspector spoke with employee and operator on proper handwashing/glove use with handling of raw and ready-to-eat foods.

Establishment advertised crab on menu/menu board but served imitation crab. On printed menus under side orders, "Crab Rangoon" is listed. Establishment only serves imitation crab. Also on menu board above ordering line under side orders, the board, per operator has a computer error and goes between displaying "Crab Rangoon" and "Krab Rangoon". Operator states new printed menus have been ordered and showed inspector example menu with correction made on the Crab Rangoon.
