Soap dispenser at handwash sink not working/unable to dispense soap, located at preparation kitchen.
Food stored on floor. Observed several cases of French fries stored on the walk in freezer floor. During the inspection employee placed French fries cases on the shelves.

No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. Observed no form, inspector emailed General Manager form.
Clean glasses, cups, bowls, plates, pots and pans not stored inverted or in a protected manner. Observed ice bin on dish rack not inverted for protection.
Cleaned and sanitized equipment or utensils not properly stored. Observed plates stored on same rack with scrub brushes. Spoke to manager about segregating items.
Dishmachine not washing/rinsing properly. Must wash, rinse and sanitize all dishware, equipment and utensils in three-compartment sink until dishmachine is functioning properly. Observed dish machine not sanitizing to proper ppm. Observed 0 ppm chlorine reading. Establishment has three compartment sink which was set up, quat reading 200 ppm. Operator will have associates use three compartment sink until dish machine is fixed.
