Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Chlorine inside dish machine sanitizer at 0 PPM manager readjusted chlorine to 50 PPM at time of inspection.
Vacuum breaker missing at splitter added to mop sink faucet.
Certified Food Manager or person in charge lacks knowledge of foodborne illnesses and symptoms of illness that would prevent an employee from working with food, clean equipment and utensils, and single-service items.
No soap provided at handwash sink. Missing at handwashing sink adjacent to fryers on front line. Manager provided soap at time of inspection.

Area manager has original certificates in his office. Photos of approved food handler certificates were emailed to store manager at time of inspection.
