Single-use gloves not changed as needed after changing tasks or when damaged or soiled. -Observed employee used her cellular phone and continue engaging in food preparation without changing gloves and wash hands. After brief education, employee then changed her gloves and wash hands.
Accumulation of dead or trapped birds, insects, rodents, or other pests, in control devices. Observed 1 dead flying insect on C02 tank.
Dead roaches on premises. -Observed 1 dead roach in prep station behind C02 tanks, 1 dead roach behind ice machine, 2 dead roach underneath front counter. Operator clean and sanitized areas.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. -Observed at 2:05 p.m. cut tomatoes cold held at 46 degrees Fahrenheit. Per operator, cut tomatoes transfer to unit at 10:30 a.m. advice operator to transfer cut tomatoes to walk in freezer for quick chill.
