Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Vacuum breaker missing on non-chemical side of threaded splitter added to mop sink.
Equipment and utensils not rinsed between washing and sanitizing. Observed employee wash pan with soap then place directly in sanitizer water. No rinse step. Manager coached employee on three compartment sink procedures.
In-use utensil stored in standing water less than 135 degrees Fahrenheit. Utensils at steam well at 127F. Manager replaced water and now temperature is 162F.
