Employee touched soiled surface and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Observed employee handling personal phone then preparing customers order for drive through with no hand wash step.
Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. All time/temperature control food for safety in steam well not holding above 135F, gravy (108F - Hot Holding); mashed potatoes (121F - Hot Holding); rice (114F - Hot Holding); beans (111F - Hot Holding), operator states steam table having an issue maintaining temperature range, operator reheated all products over 165F.
