Single-use gloves not changed as needed after changing tasks or when damaged or soiled. Observed cook push glasses up with gloved hands, and began frying fries without changing gloves.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Rice in walk-in cooler at 50f for over 4 hours. See stop sale. Chicken in reach-in cooler at 46f for 2 hours. Operator applied ice bath under chicken to rapidly cool to 43f after 10 minutes.
