See stop sale. Observed cooked rice at temperatures of 51-53F in walk-in cooler. Person in charge stated rice was cooked earlier in morning but more than 6 hours.
Stop Sale issued on time/temperature control for safety food due to temperature abuse. Observed 1/2 bag of cheddar cheese,12 pans of raw chicken, and 1 opened case of raw chicken at temperatures of 51-53F. Person in charge stated food items have remained in walk-in cooler over night and throughout the shift. A work order was placed to fix a malfunctioning walk-in cooler. Per person in charge maintenance had arrived to diagnose but had not fixed the walk-in cooler temperature.

Observed 1/2 bag of cheddar cheese,12 pans of raw chicken, and 1 opened case of raw chicken at temperatures of 51-53F. Evidence of feeding stray/wild animals - food/water bowls provided outside. Observed individual filling water bowls behind dumpster of establishment. Upon further observation area behind dumpster has approximately 7 new bowls of food for stray animals in area. Inspector informed person in charge of evidence of feeding of stray animals behind dumpster.
Food-contact surface soiled with food debris, mold-like substance or slime. Observed mold like substance and water build up at bottom of ice scoop holder.
Cold holding equipment not maintained in good repair. Do not store time/temperature control for safety food in this unit until the unit is repaired. Observed walk-in cooler with fan unit in disrepair. Ambient temperature inside walk-in cooler was at temperature of 55F.
Employee with no hair restraint while engaging in food preparation. Observed 1 employee engaging in food preparation with no hairnet.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed food debris build up on gaskets of walk-in cooler and reach in cooler doors.
Old labels stuck to food containers after cleaning. Observed old labels stuck to clean containers stored above three compartment sink. Person in charge removed labels.

No paper towels or mechanical hand drying device provided at handwash sink. Observed no paper towels at hand wash sink near to go window. Person in charge provided paper towels.
Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed ceiling tiles throughout kitchen area soiled with grease build up.
Employee personal food not properly identified and segregated from food to be served to the public. Observed a can of chips belonging to employee stored next to single service items on prep shelf under lemonade machine. Person in charge voluntarily discarded can.

Dead flies on premises. Observed 2 dead flies at front counter area. Employee swept flies and discarded.
Floor tiles missing and/or in disrepair. Floor tiles on cook line and in walk-in cooler in disrepair.
Floors, walls and/or ceilings soiled/has accumulation of debris. Floor under fryers at cook line have grease build up. Extinguisher tank attached to wall at end of fryer line, grease build up.
Garbage on the ground and/or pad around dumpster. Build up of unbagged trash at dumpster enclosure.
In-use utensil used with moist food stored in running water with insufficient velocity to flush food particles away. Water turned off on dipperwell in prep area, water 83f. Employee turned water on.
