Upon inspection, no proof of employee training available at establishment.
Equipment in poor repair. Upon inspection, observed walk-in cooler door in disrepair. Large gap observed at bottom corner of the door in which outside light can be seen from inside walk-in cooler.
Floor tiles missing and/or in disrepair. Upon inspection, observed broken/missing floor tiles on cook line and in kitchen area at establishment.
Plumbing system in disrepair. Upon inspection, observed drain pipe at handwashing sink on front line to be disconnected. Bucket observed under sink to catch water from the sink drain.
Wet mop not stored in a manner to allow the mop to dry. Upon inspection, observed mop stored with mop head on the ground in rear of establishment.
