Observed their dish machine at 25ppm chlorine. Informed the operator to call their provider and set up their triple sink.
No chemical test kit provided when using sanitizer at three-compartment sink/warewashing machine or wiping cloths. Observed no chemical tests kit for their chlorine dish machine.
Employee with no hair restraint while engaging in food preparation. Observed their prep cook working without a hair restraint. Operator had them put on a hair net.

Bathroom located inside establishment not completely enclosed with tight-fitting, self-closing doors. Observed the bathroom door not self closing. As a repeat violation the importance of following proper protocol with this violation was reinforced with the employee.
