Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Dish machine sanitizer not working, chlorine 0ppm, operator placed call for repair, dishwashing activities have been moved to 3 compartment sink where bleach is used to sanitize At 100ppm.
Nonexempt fish offered raw or undercooked has not undergone proper parasite destruction. Fish must be fully cooked or discarded. Operator unable to provide parasite destruction, salesman arrived with proof of parasite destruction for salmon.
Raw animal food stored over/not properly separated from ready-to-eat food. At walk in cooler pan containing raw hamburger patties stored over ready to eat vegetables, operator moved raw hamburger to proper position above ready to eat vegetables.
