Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Observed chlorine dish machine at 0ppm. Operator set up 3 compartment sink to sanitize dishes.
Food-contact surface soiled with food debris, mold-like substance or slime. Observed table can opener in kitchen with food debris. Operator removed can opener for cleaning.
Accumulation of lime scale on the inside of the dishmachine. Observed buildup of lime scale in dish machine.
Cutting board has cut marks and is no longer cleanable. Observed multiple cutting boards in kitchen with groves and cut marks.
Floor not cleaned when the least amount of food is exposed. Observed floors in bar area beneath equipment with a build up of soil and food debris.

In-use tongs stored on equipment door handle between uses. Observed tongs on oven door handle on cook line. Operator removed tongs.

Raw animal food stored over/not properly separated from ready-to-eat food. Observed raw shrimp stored over ready to eat shredded cheese and heavy cream in the right side cook line make table. Operator removed the shrimp.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed on the cook line in ice: Butter (65F - Cold Holding); Cut tomatoes (65F - Cold Holding); Mozzarella (55F - Cold Holding). Observed in the pizza make table: Shredded cheese (45F - Cold Holding); Sausage (45F - Cold Holding). Operator stated the food had been out for approximately 2hrs. Operator placed more ice on the cook line foods and placed ice on the make table foods.
Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Observed the vacuum breaker on the wrong end of the mop sink splitter.
Handwash sink not accessible for employee use at all times. Observed the warewashing and pizza make area hand wash sinks blocked by dirty dishes. Operator removed the dishes.
Bowl or other container with no handle used to dispense food. Observed multiple bowls and single service cups used as scoops in containers of food throughout the kitchen. Operator removed them.
Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Observed multiple employee drinks above the cook line and clean dishes in the kitchen. Operator removed the drinks. As a repeat violation the importance of following proper protocol with this violation was reinforced with the employee.
