Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Observed stew (70F - Hot Holding) on stove. Discussed hot holding with person in charge. Person in charge elected to dispose of stew as they had no intention to save it after lunch.

Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Dish machine was at 0ppm. Primed several times and it then tested at 75ppm.
Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Ice bin has black mold like substance inside of it.
