Whole fruit displayed for self-service not wrapped and no utensils provided. Ready to eat apples and grapes on buffet line - operator placed tongs.

Dishmachine not washing/rinsing properly. Must wash, rinse and sanitize all dishware, equipment and utensils in three-compartment sink until dishmachine is functioning properly. Dishwasher - downstairs(Chlorine - Not Set Up). Operator using upstairs dish machine.
In-use tongs stored on equipment door handle between uses. Downstairs oven door. Operator removed.
