Observed employee headphones and notebook on top of powder sugar container at prep area. Operator removed the items. Observed employee phone and wallet on top of container with rice. Operator removed the items.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed raw beef (63F - Cold Holding) at preparation area. As per employee was taken out of the walk in cooler to prepare 20 min ago. Employee placed product inside walk in cooler for rapid cooling.
Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Observed hose connected to spigot not vacuum breaker installed next to triple sink.
No proof of required state approved employee training provided for employee hired more than 60 days ago. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. Observed 3 food employees Cesar G, Eduardo and Ali hired for more than 60 days without proof of food handler certification.
