Drain cover(s) missing. Observed a missing drain cover in the mop sink area.

Walk-in cooler/freezer shelves with rust that has pitted the surface. Observed the shelves of the walk in cooler with rust, so the surface is no longer smooth and easy to clean.

Bowl or other container with no handle used to dispense food. Observed a small cup with no handle in rice seasoning. Manager removed the cup.
Equipment in poor repair. Ice dispenser on self service soda machine continues to dispense ice when disengaged, causing ice to spill on floor. Management has a caution sign in area to alert customers.
