Non-food grade paper/paper towel used as liner for food container. Terry cloth wiping towel was stored in a tall Cambro container on top of potato strings in water. Chef removed towel.

Dented/rusted cans present. See stop sale. Two cans of albacore tuna in dry storage observed dented, one with sharp points and one near the lip of the can.
Dishmachine not sanitizing properly. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Ran three times with same result: final rinse temperature registered 146F on calibrated thermometer probe. Dishwasher (Temperature 146F)

Raw animal foods not properly separated from each other in holding unit based upon minimum required cooking temperature. Raw chicken was stored directly above raw fish in the cooling drawers on the cook line. Chef moved to proper hierarchy for food protection. Ground beef was stored directly above whole muscle steaks and pork in in the walk-in cooler. Chef moved to proper hierarchy for food protection.
Menu contains an item made with a raw/undercooked animal food as an ingredient and description of item does not inform consumer of raw/undercooked animal food ingredient. Asian crunch salad on the menu contains Ahi Tuna with no raw identifier or reference to the Consumer Advisory. Chef began marking with a marker for use until new menus are printed.
No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. Provided forms in English, Creole and Spanish to the Chef.

Operator is not properly tracking/marking the number of days ready-to-eat, time/temperature control for safety food prepared onsite was held at refrigeration temperatures prior to freezing in order to properly date mark the food when it is thawed and held at refrigeration temperatures again.
