Dishmachine not sanitizing properly. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Testing at 0ppm, had operator change bottle of sanitizer, still not testing. Advised that the only means of ware washing should be completed in triple sink until machine is repaired.
Raw animal food stored over or with unwashed produce. Observed raw shelled eggs stored over unwashed produce. Manager put eggs on bottom shelf.

Employee touched soiled surface and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Employee handled his cell phone and continued to cut the avocados. I informed the manager and he had the employee to remove his gloves and wash his hands.
Food placed in soiled container/equipment. Cut avocado placed back into the cardboard shipping container. I had the employee to use a clean prep container.
Handwash sink not accessible for employee use at all times. Hand washing sink at the entrance to the kitchen was blocked by a cart with tortilla chips. The cart was removed.
In-use knife/knives stored in cracks between pieces of equipment. Between the large flip top reach in cooler and the table where the microwave is stored. The knife was removed to be washed, rinsed and sanitized. Ice bucket stored on the floor in the bar area under the hand washing sink. The bucket was removed and taken to be washed, rinsed and sanitized.
Raw fruits/vegetables not washed prior to preparation. Employee cutting avocados in the dishwasher area. I had the employee to wash the avocados before continuing to cut.
