Operator stated that the door was left open for delivery. Operator placed in another cooler for rapid cooling. Observed inside reach in cooler at pizza station Sliced Ham (48F - Cold Holding) as per operator since the day before. Observed also at pizza station. Shredded Mozzarella (55F - Cold Holding); Tomato Sauce (57F - Cold Holding); Shredded cheese (52F - Cold Holding). As per operator for less than 4 hrs.

Operator is not properly tracking/marking the number of days ready-to-eat, time/temperature control for safety food prepared onsite was held at refrigeration temperatures prior to freezing in order to properly date mark the food when it is thawed and held at refrigeration temperatures again. Observed lasagna no time mark after thawing inside reach in cooler.
Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Observed a bag pack and cell phone on top of single service pizza boxes at the front counter. Operator removed the items.
