Observed employee touch raw chicken then, without washing their hands or changing their gloves, touched a clean single service container for a customers order. Employee washed their hands and changed their gloves and the single service container was discarded.
No paper towels or mechanical hand drying device provided at handwash sink by the back prep area. Operator provided paper towels.

Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Observed Dishwasher (Chlorine 0ppm) because of empty sanitizer container. Operator filled new sanitizer container and dishwasher at 100ppm.
