Employee failed to wash hands before putting on gloves to initiate a task working with food. Employee at cookline handled personal phone, then put on gloves without washing hands. Inspector stopped the employee and had him wash hands and put on new gloves before proceeding to handle food.
No proof of required state approved employee training provided for any employees. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. Establishment has no proof of current food handler training for any employees.
No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. One employee working in kitchen and handling food without proof of food handler training or proof they have been informed of their health reporting responsibilities. Manager had the employee read and sign the health reporting agreement during this inspection.

Employee touched personal phone with gloved hand, changed gloves, then handled clean utensils and continued preparing food without first washing hands. Inspector stopped employee and had them wash hands and put on new gloves before continuing to prepare food.
