Employee with no beard guard/restraint while engaging in food preparation. Chicken prep station: Observed employee prepping raw chicken to fry without a beard guard.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Three compartment sink: spray hose nozzle soiled. Discussed with the operator, employee started cleaning during the inspection.
Objectionable odors in bathroom or other areas of the establishment. Observed by the large ice machine, offensive smell. Discussed with the operator. as per operator awaiting plumber to check the area.
A return visit. The problems from the previous inspection had been dealt with.

Employee touched bare body part and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Observed employee prepping raw chicken, placed inside the deep fryer, wipe apron with gloved hands, touched the handle to remove the cooked chicken from another deep fryer. Inspection coached employee. Employee washed hands properly and put on clean gloves. Also wiped of the handle to the fry basket with a sanitized towel.
Food-contact surfaces not sanitized after cleaning, before use. Do not use equipment/utensils not properly sanitized. Sanitizer Bucket (Quaternary 0ppm), observed employee wiping down the counter top of the flip top reach in cooler in the chicken prep station with a wet wiping cloth. Three compartment sink: Observed employee placing clean pans inside the sink labeled sanitizer: Triple Sink (Quaternary 0ppm), operator drained the sink and prepared new solution, Quaternary 200ppm)
Live, small flying insects found. Ware wash area, by the three compartment sink: 3 live flies resting on container lids. Dry storage area by the back door: 1 live fly on a box with mustard. Office: 1 live fly landing on the desk and chair. Also 1 live fly on the wall by the entrance to the office. Front counter: 2 live flies flying by the floor drain and ice bin containers. By the ice machine: 2 live flies flying throughout the area.
Quaternary ammonium sanitizer not at proper minimum strength for manual warewashing. Do not use equipment/utensils not properly sanitized. Three compartment sink: Observed employee placing clean pans inside the sink labeled sanitizer: Triple Sink (Quaternary 0ppm), operator drained the sink and prepared new solution, Quaternary 200ppm) Observed employee wash, rinse, and sanitize the items from the sink without sanitizer when first checked.
Employee with ineffective hair restraint while engaging in food preparation. Observed several female employees with long ponytails swinging back and forth while preparing raw chicken and removing cooked chicken from the deep fryers.
