Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Item stored in cook line warmer: hash browns (109F - Hot Holding). Manager stated hash browns placed in warmer twenty minutes prior to temperature being taken. Manager voluntarily discarded hash browns.
Food-contact surface soiled with food debris, mold-like substance or slime. Cutting boards on kitchen reach-in coolers stained.
Food stored on floor. Container of soup stored on walk-in freezer floor. Manager placed soup on shelf during inspection.

Employee stated cheese placed in cooler one hour prior to temperature being taken. Manager moved cheese to walk-in cooler to reduce temperature to 41F.
Food-contact surface soiled with food debris, mold-like substance or slime. Cutting boards on cook line reach-in coolers stained.
Equipment and utensils not properly air-dried - wet nesting. Containers stored on shelf next to kitchen hand washing sink.

Employee dried hands on clothes/apron/soiled towel after washing. Cook wearing gloves wipes hands on soiled apron then handles food and serving containers, without removing gloves, washing hands and putting on clean gloves. Discussed with Manager.
Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Jacket used by employees for walk-in-freezer stored on dry goods storage area rack. Manager removed jacket.
Employee with no beard guard/restraint while engaging in food preparation. Male employee on cook line with beard and no beard guard.

Observed in walk-in-freezer Chicken and pork in reduced oxygen package with no date mark. Manager advised that items had been delivered from their central kitchen three days earlier on Friday.
Wet mop not stored in a manner to allow the mop to dry. Observed wet mop not in use stored in mop bucket.
A return visit. The problems from the previous inspection had been dealt with.

Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Observed "Y" connector at mop sink with missing vacuum breaker on the non-chemical side of the "Y" connector.
Clean glasses, cups, bowls, plates, pots and pans not stored inverted or in a protected manner. Ice bucket not stored inverted or in a protected mannerbetween uses. Observed ice bucket used to transport ice not stored inverted or in a protected manner by ice machine. Manager removed ice buckets and set to be washed.
Covered waste receptacle not provided in women's bathroom. Observed no covered waste receptacle provided in women's outside bathroom in the handicap stall.
Establishment using sponges on cleaned and sanitized, or in-use food-contact surface. Observed establishment using sponge on in-use food-contact surface at three-compartment sink. Manager discarded sponges
Food stored on floor. Observed bulk container of oil on the floor used as a door stopper to dry goods storage area. Manager placed oil on 6" tray
Old labels stuck to food containers after cleaning. Observed old labels not removed from food containers after cleaning on the clean ware rack. Manager instructed employee to remove labels.
Single-service articles not stored inverted or protected from contamination. Observed single serve containers not inverted. Manager inverted all the single serve trays.
Unwashed fruits/vegetables stored with ready-to-eat food. Observed unwashed produce (Tomato ) stored over ready-to-eat food ( cut onions,sliced Tomato ) in walk in cooler. Manager moved Ready-To-Eat items above unwashed produce.
