Clean utensils or equipment stored in dirty drawer or rack. Observed pans lids stored between wall and gas line behind the stoves. Operator removed the lids.
Employee personal food not properly identified and segregated from food to be served to the public. Observed fruit and other food at not properly identified and segregated at reach in cooler, as per the manager from employees. Operator was informed that the employee food need to be identified and segregated.

No conspicuously located ambient air temperature thermometer in holding unit located at preparation area. Operator provided the thermometer.
Wet wiping cloth not stored in sanitizing solution between uses. Observed wet wiping cloth stored on preparation table. Operator moved the wet wiping cloth to the three compartment sink.
Wood utensils stored upright with the food-contact surface up next to stoves.
