As per cook, product out at room temperature for 3 hours. Chef placed product in ice bath. Second temperature taken, 43F. Observed Raw fish (62F-Cold holding), Cooked potatoes (46-Cold holding), Alfredo (67F-cold holding) at reach in cooler at cook line. As per cook, for 3 hours. Cook submerged containers in ice.

Stop sale was issued.

Observed chlorine sanitizer was +/- 400 ppm. Employee discarded it and fix solution to 100 ppm.
