Single-use gloves not changed as needed after changing tasks. Cook placing raw shrimp on grill gloves touching raw shrimp and then proceeded to plate ready to eat buns without washing his hands and changing his gloves. Cook went to wash his hands and changed gloves.
Commercially processed reduced oxygen packaged mahi mahi bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. Mahi Mahi 36F removed from freezer this morning. Cook removed packaging.
In-use utensil used with moist food stored in running water with insufficient velocity to flush food particles away next to ice cream freezer. Operator turned water on.
