Employee with no hair restraint while engaging in food preparation. No hair restraint for employee preparing sandwiches.
In-use utensil stored in standing water less than 135 degrees Fahrenheit. Two inserts with utensils in flip top reach in cooler in standing water at 112F and 106F. Operator removed water.
Wiping cloth quaternary ammonium compound sanitizing solution not at proper minimum strength. Wiping cloth solution at coffee prep area tested at 0ppm quaternary ammonium. Operator states the chemical system has been inconsistent in dispensing and has already made a service call for repair.
