Handwash sink not accessible for employee use at all times. Observed : the kitchen hand wash sink was blocked with the handles from a large stock pot and a cutting board on the stock pot. These were turned sideways to allow access to the hand wash sink.
Buildup of food debris/soil residue on equipment door handles. Observed : multiple handles with oil or debris. The staff started to wipe the handles.

Floor tiles missing and/or in disrepair. Observed : on the cook line floor tiles a cracked and missing.
In-use wet wiping cloth/towel used under cutting board. Observed : wet cloth under a cutting board on the back prep table. The board was removed to the 3 compartment sink.
Water leaking from pipe and/or faucet/handle. Observed : the 3 compartment sink faucet continues to drip after the handles are turned to the off position.

Handwash sink not accessible for employee use at all times. Observed : large cutting board obscured full access to the kitchen hand wash sink. The board was adjusted.
Ready-to-eat, time/temperature control for safety food prepared onsite and held more than 24 hours not properly date marked. Observed : sweet potato in curry, made yesterday, no date, in the cook line glass front reach-in cooler. ALSO, the 1/2 and 1/2 in the front line reach-in cooler has no open date. Dates were added.
Bowl or other container with no handle used to dispense food. Observed : in the cook line reach-in cooler, a handless bowl used to scoop rice. The bowl was removed.
Buildup of food debris/soil residue on equipment door handles. Observed : multiple make table door handles have food debris build up.
Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Observed : a purse with a gold colored strap, next to the walk-in cooler, stored over the microwave and clean soup bowls.
Equipment and utensils not washed, rinsed and sanitized in the correct order in three-compartment sink. Do not use dishes/equipment not properly sanitized. Observed : wash and rinse sinks set up, no sanitize, sink as staff washed a dish. Sanitizer sink now set up with 100 ppm chlorine.
Ice buildup in reach-in freezer and/or walk-in freezer. Observed : the glass reach-in freezer in the back kitchen, has ice/frost buildup inside the left door.
Reach-in cooler/freezer shelves/lid with rust. Observed : the large chest freezer has a rusty lid.
Stored food not covered. Observed : frozen food and stored (below 41°]time/temperature control for safety food not covered in multiple cold holding units. Staff started wrapping the food that was stored and not currently in use.

Establishment has no approved Alternative Operating Procedure. Observed: prep staff put together a Summer roll(All ready to eat items), no gloves. The manager retrained the employee to wear gloves.
Nonfood-grade bags used in direct contact with food. Observed: printed non food grade badges in use to store frozen meat, in the chest freezer.
Required employee training expired for some employees. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. Observed 1 expired employee.
Equipment and utensils not washed, rinsed and sanitized in the correct order in three-compartment sink. Do not use dishes/equipment not properly sanitized. Observed: no sanitizer step in the washing of dishes during the start of the inspection. The manager corrected the employee.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed: egg rolls in the back wall reach-in cooler 85F. These were cooked earlier, and had just been put in the reach-in cooler. These were placed in the reach-in freezer and dropped 8F in 12 minutes. ALSO, tofu in the front line reach-in cooler, 60F. This had been out in and out of the reach-in cooler during lunch. Ice was added and dropped 10F in 9 minutes.
Commercially processed ready-to-eat, time/temperature control for safety food opened and held more than 24 hours not properly date marked after opening. Observed half n half in the reach-in cooler at the front, no date. Yesterdays date was added.
Food-contact surface soiled with food debris, mold-like substance or slime. Observed: the food contact surface of a colander in the back wall reach-in cooler is soiled. ALSO, the cutting boards are stained on the cook line.
Handwash sink not accessible for employee use at all times. Observed: the kitchen hand wash sink with a boiling 35 gallon pot tight on one side and 3 5 gallon buckets on the other side. The buckets were moved.
